Forms for Class Action Lawsuit

Contingent Fee Agreement - Establishes agreement between clients and the law firm Macuga & Liddle, P.C.

Cover Form - Provides contact information and indicates which additional forms (A-D) you will need to complete.

Form A - Home Survey - to be completed by a household claiming odors/dust at their home.

Form B - School Survey - to be completed by a those claiming odors/dust at their child's school.

Form C - Work Survey - to be completed by area employees claiming odors/dust at their work.

Form D - General Survey - to be completed by individuals claiming odors/dust at locations other than their home, school and/or work.

Forms for Ongoing Odor Reporting

Odor Log - Used for recording and reporting hydrogen sulfide odor events.

Odor Log Instructions - Instructions for using reporting hydorgen sulfide odors using either the Odor Log form or the telephone. These are the same instructions that are presented on the Smell It, Tell It page.

Documentation Affidavit - A written declaration of an odor problem or witnessed event which may be sworn under oath before a notary public.

Health Questionaire - Used to record any illnesses you have developed since 1994 which may be related to the hydrogen sulfide problem.
